(ACC) Request FormPlease complete the following Architectural Control Committee (ACC) request form if you are planning any outside improvements of your home. Attach a diagram of the intended improvement in detail including material, height, color, placement of improvement and accompanying equipment (if applicable). Send to ACC c/o Wellington, 3520 Furlong Drive East, Flower Mound, TX 75022, email to wellingtonhoa@verizon.net, or FAX to (972) 539-3022. You will receive a written response to your request within thirty (30) days of completed receipt of request by ACC.
Article X, section 10.02 states: In connection with the submission of such plot plan, construction plans and specifications, and landscaping and grading plans, the Committee may require that the submitting party pay up to $250.00 per submission, which shall be payable to the Committee, or if the Committee elects, a representative designated by the Committee to review such plans and specifications.
Submission of fee for such plans or specifications is not a guarantee of Committee approval of plan or specific request.

It is necesssary to include the appropriate supporting items (see Architectural Control Guidelines under Docs/Info/Bulletins for more information). Any hardcopy paperwork submitted to support this application may not exceed 8.5 x 11 inches in size.

- Color, paint or stain samples
- Builder pool plans
- Architectural drawings or photos of similar projects
- Lot survey to indicate easements and setbacks
- Site plan showing location of proposed project
- Description of materials (masonry, roofing, wood, plant materials, etc.)
- Dimensions of improvements (height, width, etc.)
- Placement of accompanying equipment (if applicable)
- Access point (if applicable)

AGREEMENT:
I certify that all materials submitted in this application are true and correct. I understand and agree that no work may be performed prior to or in deviation from the terms of a written approval letter submitted by the ACC. I agree to be bound by the ACC rules and standards as written in ARTICLE X, Section 10.02 of the Declaration. I acknowledge that my Association dues account is current.
